All requests to be placed on the same team as another player must be submitted in this form. Each player is permitted one mutual request, meaning the other player must also submit a matching request. If you wish to request that your child not be placed on a specific team, this must also be submitted here, along with a clear and valid reason for the request. All requests will be reviewed, but approval is not guaranteed.
